Our Dental Membership Plan in Spring Hill

No Dental Insurance? No Problem!

Our practice offers a low-cost, in-house dental plan for a nominal membership fee. Our plan includes preventative dental care and a discount on restorative work such as fillings and crowns.

Patients who participate in our In-House Dental Savings plan experience:

• * No Yearly Maximum
• * No Deductibles
• * No Claims
• * No limitations for pre-existing treatment
• * No Waiting Periods
• * FREE Consultations

** You do not receive a membership card as this plan is only available with our office. We will keep your effective date on file.

Program Guidlines:

• * Membership fees are due the day of joining program.
• * NO Refunds of premiums will be issued at any time.
• * Children listed on plan must live in household or be a full time student.
• * Cosmetic procedures such as Botox or Whitening are NOT included in the plan.
• * Memberships are good for 1 year.

Dental Exclusions & Limitations:

This program is a discount plan, not dental insurance plan. It cannot be used:

• * In conjunction with a dental insurance plan.
• * For procedures performed outside the office.
• * For procedures that are referred to a specialist.
• * For cost of dental care under workers compensation.
• * For any dental products such as Sonicare brushes, teeth whitening, Botox, etc.

In House Membership Fees:

$450 – Adult

$400 – Children up to age 18

• * One set of bitewing x-rays ($69 Value)
• * One panoramic x-ray every 3 years ($121 Value)
• * Two regular dental cleanings ($198 Value)
• * Two dental exams ($108 Value)
• * Two Emergency Exams ($162 Value)
• * One fluoride treatment for adults ($30 Value)
• * Two fluoride treatments children ($60 Value)
• * 10% Discount on Orthodontic Services
• * 20% Discount on all treatment including sealants, fillings, crowns, bridges, periodontal treatment, extractions, dentures and night guards.
